Secondary Infertility Treatment in India

Welcome to Secondary Infertility Treatment
Most people believe that once you've had one child, you've proven yourself to be fertile and therefore will have no problems conceiving again in the future. Unfortunately, this is not true. Secondary infertility is a very common problem. In fact, it is so common that it accounts for a much as 60% of infertility cases. Yet, many people fail to see this type of infertility as a problem.
What Is It?
Secondary infertility is usually defined as the inability to conceive or carry a pregnancy to term after successfully and naturally conceiving one or more children. According to a 1995 survey by the National Center for Health Statistics, 3.3 million American women were experiencing secondary infertility. This showed an increase from 1988, when 2.7 million women were affected by secondary infertility.
Unlike those experiencing primary infertility (when a childless couple is unable to conceive), couples that are affected by secondary infertility are much less likely to get infertility treatment. This is usually due to the misconception that once you're fertile, you'll always be fertile. Medical professionals who do not have proper training in infertility issues often perpetuate this false belief. It is not uncommon for a couple to be told to just keep trying and that eventually it will happen. While this may be true in some cases, in many others, advice like this only adds to the frustration and heartache experienced by a couple having fertility problems.
Why Now? Reasons for secondary infertility tend to be the same as reasons for primary infertility. Since you were last pregnant, you or your partner may have had an infection, gained some weight, or started eating fewer healthy foods. These seemingly small variances in your life can have large repercussions on your reproductive health. Additionally, if it has been a few years since you last had a child, your egg quality may have begun decreasing or your partner's sperm may not be what it once was. Abnormalities with sperm and ejaculate are frequently cited as causes of secondary infertility. Other common explanations for secondary infertility include:

Should I See A Doctor? It is always a good idea to seek out a fertility specialist if you have troubles conceiving. If you are under 35, have been having regular, unprotected sex for a year and have not been able to conceive, make an appointment with your doctor or a fertility specialist. If you are over 35, then start investigating the issue after six months of regular, unprotected sex. However, there are times when you should make that appointment with the specialist sooner.
If you experience two or more miscarriages, have irregular periods, have especially painful periods, an unusual increase or a burning vaginal discharge, or if your partner is experiencing a decrease in his sex drive, painful ejaculations, or impotence, then make an appointment sooner. These are all indications of medical problems that need attention before a successful conception can occur.
Relationships Because secondary infertility is often unrecognized as a problem, many couples find it hard to receive the proper support from their family and friends. Some couples may even find that people criticize them as seeming ungrateful for the child or children they already have. You may find it necessary to sit down and discuss the issue with those closest to you so that they can understand why you need their support so much right now.
While fertility problems can always put a strain on a relationship, secondary infertility can be especially stressful. The ways in which people deal with the situation can vary. Communication can breakdown when one partner does not want to discuss the issue at all, causing the other to feel shut out. Whether or not treatment for the infertility should be pursued, and how much to spend can other areas of contention for couples.

It may be beneficial to seek out therapy to help you and your partner deal with the emotions that secondary infertility brings. Joining a support group for people experiencing secondary infertility can help you realize that you're not the only one going through this difficult time. Support groups have been shown to be very effective in helping people cope during a difficult time.
In addition to dealing with your own problems of infertility Treatments and your relationship, you will also need to address the issue with your child in an age-appropriate manner. Children are very receptive to the world around them, so it is unrealistic to think that your child will not notice the tense atmosphere at home. Talk about your feelings and encourage your child to express herself on the issue.
With proper support and guidance, you and your partner will be able to make the right choices for your family.

Sperm freezing Treatment in Delhi | Dyanamic Fertility Center in Delhi

Sperm freezing and storage is the procedure whereby sperm cells are frozen to preserve them for future use. Many situations that could interfere with your future fertility. These may include:

  • Vasectomy.
  • High-risk occupations or sports.
  • Diabetes, which may cause erectile difficulty.

For each treatment cycle, the success rate varies depending on the age of the woman. If the woman is:
1. Under 35, the success rate is around 19%.
2. 35–39, the success rate is around 15%.

Secondary Infertility Treatments in Delhi, India

Secondary infertility Treatments occurs when a woman or man who has previously been able to conceive isn't able to the next time around. It is a very real problem that many people may not understand as they view the couple as "already having a child."Secondary infertility, or not being able to conceive again after having one child, is more common than you might think! Many women suffer from secondary infertility and at this point, modern medicine has no answers.

However, there is hope and you do have some control of your chances of eliminating secondary infertility naturally. One of the most common causes is problems regarding the ovulation of a woman. Sometimes, pregnancy and childbirth affects the menstrual cycle and the ovulation becomes abnormal thus making it very difficult to fertilize the egg. Uterine fibroids or benign tumors in the uterus can also cause this problem. When these tumors are present, conceiving gets difficult. Endometriosis is also a common cause of this problem. This is when abnormal cells suddenly grow outside of the uterus. This condition does not have any symptoms and the main cause is still unknown.

Percutaneous Epididymis Sperm Aspiration (PESA) in Delhi, India

The single winning sperm was the one that entered the egg first. The modern infertility treatments intracytoplasmic sperm injection (ICSI) and in vitro fertilization (IVF) now make it possible to extract a single sperm and use it to directly fertilize an egg. The latest surgical sperm retrieval techniques combined with ICSI and IVF also mean that infertility can be overcome even in men who produce hardly any sperm. A low sperm count is a common cause of male infertility but if a man produces even the tiniest number of sperm in his semen, these can be collected fairly easily. Men who ejaculate no sperm, because of blocked tubes in their testes, or because of a genetic condition that prevents their sperm being released, require some form of surgical sperm retrieval to enable intracytoplasmic sperm injection to take place.

·         PESA - Percutaneous Epididymis Sperm Aspiration (can be done in the office).
The success rate will depend very much on the IVF live birth success rates of the particular in vitro fertilization laboratory. IVF success rates are also dependent on the age of the female partner - even in these "male factor" cases after vasectomies. We would generally expect the success rates to be about the same as (or somewhat higher) the rates for other couples with a female partner of the same age going through IVF.

Dr Nalini Gupta is also visiting senior consultant at Miracles Fertility and IVF clinic Gurgaon and Director at Harmony-A center for reproductive medicine and Infertility at Park Hospital Gurgaon.

Treatment of TESA in Delhi

Sperm can be aspirated with a needle from the testicle or from the vas deferens (a structure right next to the testicle that also contains sperm). The man is given some drugs to sedate him and some local anesthesia is also used to numb the area. Then a small needle is inserted and sperm is aspirated from either the epididymis or the testicle. There should be no severe pain. The procedure generally takes about 30 minutes. The different sperm aspiration and extraction procedures have long names and also short names (acronyms):

·         TESA - Testicular Sperm Aspiration (can be done in the office)
·         TESE - Testicular Sperm Extraction (involves a small incision and snipping off some tissue from inside the testicle - often done in a hospital or free-standing surgicenter).
Sperm extraction is being performed more and more by non-urologists (called andrologists) who are actually either internists or obstetrician-gynecologists. It stands to reason that these non-urologists prefer TESA, given that they are not surgically trained. There has always been debate, however, as to which procedure is “better” at obtaining sperm for successful intracytoplasmic sperm injection.

Dyanamic Fertility & IVF centre leading Dr. Nalini Gupta will be understanding and appreciation of the diverse needs of international patients enables us to offer them a wide range of infertility treatments. She is having strong medical ethics and is having a patient friendly approach in her work.

In vitro fertilization or IVF Treatment in Delhi

In vitro fertilization or IVF is an advanced process of fertilisation, which involves the combining of an egg with a sperm in a laboratory dish. This is followed by the implantation of the embryo(s) [a developing unborn offspring formed by the combination] in your uterus. Being one of the most successful and widely opted methods of assisted reproductive technology, IVF is considered to be highly advantageous for couples who seek a child but have fertility issues.

Here’s why:
1. IVF proves to be a boon for couples who are normally unable to conceive: Couples who have fertility issues generally opt for IVF to fulfil their wish of having a child. IVF works against fertility problems like blocked fallopian tubes, PCOS, endometriosis (a condition where a fraction of the womb lining grows outside it), premature ovarian failure or menopause among women and unusual sperm size, low sperm count, and inadequate sperm mobility among men.
2. It has been in use since long and is proven to be safe: IVF was first used in 1978 to give life to the baby Louise Brown, safely. Since then, the enhancements in technology and techniques have resulted in large numbers of successful cases, where IVF has proved to be a safe method for conceiving.
3. It has higher success rates in comparison to other types of technologies used for assisted reproduction: Though various types of assisted reproductive technologies like IUI (Intrauterine insemination) are available for couples seeking a child, the success rate and effectiveness of IVF is the highest. This is due to the fact that IVF has undergone significant levels of enhancement over the years in comparison to other such techniques.

4. It proves to be of great help to same-sex couples and single women: Same-sex couples and single women are hugely benefitted through IVF as it helps them in becoming parent(s) by using donor sperms.
5. It can help you to eliminate genetic disorders: IVF helps in preventing the conceived child from inheriting any general disorders present in the parents through the process of pre-implantation genetic diagnosis (PGD – a reproductive technology that accompanies an IVF cycle to detect genetic diseases in the embryo before the implantation happens). Moreover, opting for pre-implantation genetic screening (PGS) increases the probability of success as it entails a screening of the embryos that helps in identifying chromosomal disorders like Down’s syndrome.

Egg Donation in Delhi

Egg donation is the process where a woman donates eggs which will be mainly due to purposes of reproduction by another woman or may be for research. Many clinics take these donations with a view of helping women who are facing fertility complications. When searching for an egg donor the intended parents will usually want to find a donor who has similar characteristics similar to theirs, such as ethnicity, hair color, eye color, and body type. Egg donation refers to the process where a woman donates eggs for purposes of reproduction by another woman or even for purposes of research. Many clinics across the world today take these donations with a view of helping women who are facing fertility complications. The types of egg donors include designated, undesignated and IVF. Designated donors are those ones who donate eggs to friends and relatives that are in need. On the contrary, undesignated donors are generally anonymous and unrelated. IVF refers to a case where the egg will be fertilized by a sperm outside the body.

Blastocyst Transfer And IVF Treatment in Delhi

Blastocyst transfer is very common in IVF treatment. Embryos need to grow to later stages in culture. Media or culture fluid has been developed which contains everything they need for the later stages. Blastocysts are naturally to be found in the womb after in vivo conception. They travel down the Fallopian Tube dividing and reach the womb at a stage ready to implant. In IVF, day 3 embryos are transferred into the womb where they wait and grow until they are ready to implant. It is not possible to transfer them into the Fallopian Tube where they would naturally be at this stage.

In early embryos only 1 or 2 cells can be extracted without causing the embryo too much harm. But due to the larger number of cells of a Blastocyst (around 100), more cells can be used to check for abnormalities giving a more accurate result with less impact on the embryo. So if the intention is to use PGS, then a Blastocyst transfer may offer a better outcome.

IVF Surgeon in Delhi

IVF is short for In Vitro Fertilization which was introduced to the USA over 32 years ago.IUI (intra uterine insemination) infertility treatment with or without egg production stimulation medication. The female egg is not touched. The partner gives a fresh semen sample and it is washed and inserted into the womb with a small catheter during ovulating. Fertility is a huge relationship stressful, but the decision should always be made mutually when it comes on the treatment. For IVF treatment what you really need is significant physical, emotional, financial, and time commitment.

According to various reports, the success rates of IVF are,
·        Women’s having under age of 35 is 45 – 47.1%.
·        Women’s age of 35 – 37 is 36.1 - 37.9%.
·        Women’s age of 38 – 40 is 24 – 28.5%.
·        Women’s age of 40- 42 is 16.3 – 18%.
·        Women’s age above 42 is 6.1- 10%.

Traditional IVF treatment is universally, the most popular form of the treatment, however, there are a number of variations from the traditional one, and this is to accommodate people who have certain religious beliefs, or because specialists feel that there is a better chance of a successful treatment.

Frozen Embryo Transfer (FET) in India

Frozen Embryo Transfer (FET) is considered a non-invasive or minimally invasive type of procedure. Frozen embryos can be thawed and then implanted into your uterus. If you have ovarian or ovulation problems, then FET might be a good option for you when you are considering your infertility treatment options.

Depending on what your fertility problems are, this simple treatment can help you. If your problems are ovulation or ovarian based then simply having the embryo placed within your uterus can help you to become pregnant. If your problems are more uterus based, as in not being able to carry a baby to full term or any other kind of disorder then F.E.T. may not be able to help.

There are few different ways that a frozen transfer can occur and one method is by monitoring a woman for her menstrual cycle. The other method is simply monitoring ovulation and placing the eggs inside her uterus after ovulation. This will help supplement the other part of a woman's cycle with more progesterone. This helps to create a cycle as it gives progesterone and estrogen to help suppress Lupron. It then performs what is called an artificial cycle. This is going to be a process that is not as complicated as IVF. This is because egg retrieval is eliminated as well as stimulating more eggs.
